At your visit, we will need:

• Photo ID
• Referral, if required by your insurer
• Check, credit card, or cash for co- payment, if required by your insurer

To expedite your check-in, we ask that you complete the following forms prior to your appointment. These forms are available for you to download and print below:

• Receipt of Notice of Privacy Practices Written Acknowledgement Form
   
• Medical History Form
