Welcome to your new patient portal.  Here you can conveniently message your providers, pay your bill, access your notes, photos, and some labs.  Instructions to access your portal are below.

Instructions to access your portal

To access your portal for the first time, click Forgot Password.  Enter your username, which is the email address you used when registering for your visit.  Then enter your date of birth and last name.   The system will send a password reset link to your email.  Once you’ve setup your password, you can access your portal from anywhere using your smart phone or computer.

Please note that the patient portal is not Telehealth. For new, or existing medical conditions that require evaluation, you will need to schedule an office visit.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. We look forward to your first visit.

At Your Visit We Will Need:

Your Insurance Card(s)
Referral, if required by your insurer
Check, credit card, or cash for co- payment, if required by your insurer

Downloadable Patient Forms

Forms need to be printed, filled out, and brought with you to your visit.
Do not upload them to your patient portal.

We strive to get you the medications you need in a timely, efficient manner. Many prescriptions should be available by the end of the day. However, we ask that you allow 72 hours from your appointment time for availability. Some medications require a prior authorization by your insurance. This can add significant delay to medication arrival. Our office will work diligently with your pharmacy and insurance to get your medication quickly approved.

If an emergency situation occurs after office hours, please call 502-583-1749.
Our answering service will contact the physician on call, and you will be contacted as promptly as possible.
